Oasis Therapy Services in Madison, Wisconsin is seeking an Occupational Therapist to provide one-on-one rehabilitation in an assisted living setting. This PRN role focuses on maintaining independence through ADLs and mobility, with emphasis on safety and individualized care.

The OT will document progress in Net Health, educate residents and caregivers, and collaborate with the care team to optimize outcomes. A supportive atmosphere promotes professional growth and compassionate service.

Qualificações

• Active and current Wisconsin Occupational Therapist license.
• Graduate of an accredited Occupational Therapy program.
• Current CPR certification.
• Strong communication, organization, and documentation skills.
• Geriatric, assisted living, skilled nursing, or rehabilitation experience is preferred.

Responsabilidades

• Evaluate residents and develop individualized treatment plans.
• Provide therapy focused on ADLs, mobility, safety, and independence.
• Monitor progress and adjust treatment plans as needed.
• Educate residents and caregivers on exercises, adaptive techniques, and equipment.
• Complete accurate Net Health documentation and collaborate with the care team.
