Job summary

Total Care Therapy is seeking an Occupational Therapist in Avon, IN to assess residents and develop personalized treatment plans aimed at improving daily living activities, cognition, and safety. The role emphasizes collaboration with an interdisciplinary team and hands-on leadership of therapy care teams.

You will supervise COTAs, document thoroughly in EMR, and participate in a care model focused on geriatrics within assisted living and skilled nursing environments.

Qualifications

• Current State Occupational Therapist (OT) License.
• Current CPR/BLS Certification.
• Valid driver's license, reliable transportation, and auto insurance.
• Experience in geriatrics, home health, assisted living, or skilled nursing preferred but not required.

Responsibilities

• Evaluate residents and establish individualized occupational therapy plans of care.
• Provide skilled occupational therapy interventions to improve ADLs, IADLs, upper extremity function, cognition, balance, coordination, and independence.
• Assess and recommend adaptive equipment and environmental modifications.
• Reassess patient progress and modify treatment plans as appropriate.
• Supervise COTAs in accordance with state regulations.
• Complete timely and accurate documentation using EMR.
• Educate residents, caregivers, and facility staff on adaptive techniques and safety.
