Job summary

Total Care Therapy is seeking an Occupational Therapist to join our Columbus, Ohio team. You will evaluate residents and develop individualized treatment plans to improve independence in daily activities, cognition, and safety.

You will lead OT interventions and collaborate with COTAs and the interdisciplinary team to support older adults in assisted living and independent living communities. Responsibilities include documentation in EMR, environment and equipment recommendations, and ongoing

Qualifications

Current State OT license and CPR/BLS certification.
Driver's license and reliable transportation.
Experience in geriatrics or related settings preferred but not required.

Responsibilities

Evaluate residents and establish individualized occupational therapy plans of care.
Provide skilled OT interventions to improve ADLs, IADLs, upper extremity function, cognition, balance, and functional independence.
Assess and recommend adaptive equipment and environmental modifications.
Reassess progress and modify treatment plans as appropriate.
Supervise COTAs in accordance with state regulations.
Complete timely and accurate EMR documentation.
Educate residents, caregivers, and facility staff on safety and adaptive techniques.
Collaborate with interdisciplinary team for coordinated, patient-centered care.
