Job summary

Total Care Therapy in Fort Wayne, IN is seeking a part-time Occupational Therapist to evaluate residents and develop individualized treatment plans to improve ADLs and cognition, with a focus on independence and safety. You will work with an interdisciplinary team in assisted living and skilled nursing environments, provide skilled OT interventions, supervise COTAs, and document care using EMR.

Qualifications

• Current State OT license is required.
• CPR/BLS certification is required.
• Valid driver's license with reliable transportation and auto insurance.
• Experience in geriatrics, home health, assisted living, or skilled nursing preferred but not required.

Responsibilities

• Evaluate residents and establish individualized OT plans of care.
• Provide skilled OT interventions to improve ADLs, IADLs, upper extremity function, cognition, balance and safety.
• Assess and recommend adaptive equipment and home/environmental modifications.
• Reassess progress and modify treatment plans as appropriate.
• Supervise COTAs in accordance with state regulations.
• Complete timely documentation using the EMR.
• Educate residents and caregivers on adaptive techniques and fall prevention.
• Collaborate with interdisciplinary team for coordinated patient-centered care.
