Job summary

FOX Rehabilitation is seeking a licensed Occupational Therapist in Muncie, IN to deliver one-on-one care in private residences or senior living communities. This part-time role supports autonomy, flexible scheduling, and meaningful patient outcomes, with potential telehealth components to extend access.

You will evaluate, plan, and provide evidence-based therapy to help older adults improve mobility and independence, while collaborating with an interdisciplinary team and documenting in FOX’s EMR

Qualifications

• Valid Occupational Therapy license in the state(s) of practice, or eligibility to obtain.
• Degree from an accredited Occupational Therapy program.
• Basic computer and EMR proficiency.
• Current CPR certification.

Responsibilities

• Provide one-on-one Occupational therapy to older adults wherever they call home (private residences, senior living communities, etc.).
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions to support wellness, longevity, mobility, and independence.
• Document effectively within FOX’s EMR system per regulatory standards.
• Collaborate with an interdisciplinary team (PTs, OTs, SLPs, Fitness Specialists).
• Educate patients, caregivers, and staff to promote safety and aging in place.
• Provide telehealth / virtual care when appropriate to extend access.
