Job summary

Renewal Rehab is seeking a Licensed Occupational Therapist to provide patient-centered care for seniors in skilled nursing and post-acute facilities. In this onsite role, you will evaluate residents, develop individualized treatment plans, and deliver evidence-based interventions to improve ADLs, mobility, cognition, and safety.

You’ll document in EMR, collaborate with an interdisciplinary team, and educate families and caregivers.

Qualifications

• Must hold an active OT license in the state.
• Experience in skilled nursing and post-acute settings preferred.
• Ability to document in EMR and collaborate with interdisciplinary team.

Responsibilities

• Evaluate residents and assess functional abilities.
• Develop individualized treatment plans.
• Deliver evidence-based interventions for ADLs, mobility, cognition, and safety.
• Document in EMR and coordinate with the care team.
• Educate families and caregivers on care plans.
