Job summary

Renewal Rehab is seeking a Licensed Occupational Therapist to provide patient-centered care for seniors in skilled nursing and post-acute facilities. This onsite role involves evaluating residents, developing individualized treatment plans, and delivering evidence-based interventions to improve ADLs, mobility, cognition, and safety.

You will document in EMR, collaborate with an interdisciplinary team, and educate families and caregivers.

Qualifications

• Must hold an active Occupational Therapist license.
• Experience in skilled nursing and post-acute facilities is preferred.

Responsibilities

• Evaluate residents to determine functional status and needs.
• Develop individualized treatment plans to improve ADLs, mobility, cognition and safety.
• Deliver evidence-based interventions to enhance daily functioning and quality of life.
• Document care, progress, and outcomes in the EMR.
• Collaborate with an interdisciplinary team to coordinate care.
• Educate families and caregivers on treatment plans and home strategies.
