Job summary

Renewal Rehab seeks a Licensed Occupational Therapist to provide patient-centered care for seniors in skilled nursing and post-acute facilities. Onsite role evaluating residents, building individualized plans, and delivering evidence-based interventions to improve ADLs, mobility, cognition and safety.

You will document in EMR, work with an interdisciplinary team, and educate families and caregivers, contributing to daily improvements in quality of life and functional outcomes for seniors.

Qualifications

• Active OT license in Michigan.
• Experience delivering patient-centered care in SNF or post-acute settings.
• Strong documentation and interdisciplinary collaboration skills.

Responsibilities

• Evaluate residents to determine needs and goals.
• Develop individualized treatment plans for ADLs, mobility, cognition, and safety.
• Deliver evidence-based OT interventions to improve functional outcomes.
• Document care in EMR and maintain accurate records.
• Collaborate with an interdisciplinary team and educate families/caregivers.
