Job summary

Renewal Rehab is seeking a Licensed Occupational Therapist to provide patient-centered care for seniors in skilled nursing and post-acute facilities. This onsite role involves evaluating residents, developing individualized treatment plans, and delivering evidence-based interventions to improve ADLs, mobility, cognition, and safety.

You will document in an EMR, collaborate with an interdisciplinary team, and educate families and caregivers.

Qualifications

• Active state licensure as an Occupational Therapist (OTR)
• Experience in skilled nursing or post-acute care is preferred
• Excellent communication with residents, families, and care teams

Responsibilities

• Evaluate residents to determine functional status and goals.
• Develop individualized treatment plans focused on ADLs, mobility, cognition, and safety.
• Deliver evidence-based interventions to improve daily functioning and independence.
• Document progress and outcomes in the EMR; collaborate with an interdisciplinary team and educate families/caregivers.
