Job summary

Renewal Rehab seeks a Licensed Occupational Therapist to provide patient-centered care for seniors in skilled nursing and post-acute facilities. In this onsite role, you will evaluate residents, develop individualized treatment plans, and deliver interventions to improve ADLs, mobility, cognition and safety.

You’ll document in EMR, collaborate with an interdisciplinary team, and educate families and caregivers.

Qualifications

• Licensed OT in Illinois (OTR/L) in good standing.
• Experience delivering therapy in SNF or post-acute settings is preferred.

Responsibilities
