Qualifications

Active and unrestricted California Occupational Therapist (OT) License.
Strong clinical assessment and treatment skills.
Excellent communication and interpersonal abilities.
Preferred experience in Skilled Nursing Facilities (SNF) or post-acute care.

Responsibilities

Evaluate residents and develop individualized treatment plans.
Provide skilled, evidence-based occupational therapy interventions.
Help residents improve daily living skills, safety, mobility, and independence.
Collaborate with Physical Therapists, Speech-Language Pathologists, nursing staff, physicians, and interdisciplinary care teams.
Educate residents, families, and caregivers regarding treatment goals and progress.
Maintain accurate, timely, and compliant documentation.
Monitor progress and modify treatment plans as needed to achieve outcomes.
