Job summary

Napa Post Acute in Napa, CA is seeking an Occupational Therapist (OT) to join our rehab team. You will evaluate residents and develop individualized treatment plans to improve daily living and independence. You will collaborate with PTs, STs, nursing and other team members, while keeping documentation current and clear for residents and families. Qualifications

• Current California Occupational Therapy license is required.
• Experience in Skilled Nursing or post-acute settings preferred, but not required.
• Excellent communication and teamwork skills.
• Compassionate, patient-centered approach to care.

Responsibilities

• Evaluate residents and develop individualized treatment plans.
• Provide occupational therapy services to improve function and daily living skills.
• Collaborate with PTs, STs, nursing, and interdisciplinary team members.
• Complete timely and accurate documentation.
• Educate residents, families, and staff on treatment goals and care strategies.
• Ensure compliance with regulatory and clinical standards.
