Qualifications

• Licensed OT or license eligible in the state of practice.
• Graduate of an accredited Occupational Therapist program.
• Interest or experience in skilled nursing, long-term care or geriatric rehabilitation.
• Passion for patient-centered care and improving quality of life for older adults.

Responsibilities

• Evaluate patients and develop personalized care plans focusing on independence and safety.
• Deliver therapy through activities, ADL training, and adaptive techniques.
• Assist patients with daily tasks such as dressing, bathing, grooming, and meal prep.
• Collaborate with interdisciplinary teams, caregivers, and nursing staff to ensure strong outcomes.
• Track patient progress and maintain accurate clinical documentation.
