Qualifications

• Licensed OT in the state of practice or eligible for licensure.
• Graduate of an accredited OT program.
• Interest or experience in skilled nursing, long-term care or geriatric rehabilitation.
• Passion for patient-centered care and improving quality of life for older adults.

Responsibilities

• Evaluate patients and develop personalized care plans focused on functional independence, safety, and quality of life.
• Deliver care through ADL training, therapeutic activities, and adaptive techniques.
• Help patients perform daily tasks such as dressing, bathing, grooming, and meal prep.
• Partner with interdisciplinary teams to support outcomes and continuity of care.
• Track patient progress and document clinically; adapt treatment plans as needed.
