Job summary

Majestic Care of Toledo is seeking an Occupational Therapist to evaluate, plan, and implement targeted OT treatments for residents, with a focus on ADLs, functional mobility, and independence in a geriatric setting. The OT collaborates with the care team to optimize outcomes and quality of life.

You will supervise COTAs, document progress, educate families, and coordinate with an interdisciplinary team to ensure high-quality, resident-centered care, while upholding HIPAA and facility standards.

Qualifications

• Current state licensure as an Occupational Therapist.
• Minimum 2 years of experience in occupational therapy, preferably in a long-term care or geriatric setting.
• Supervise COTAs and educate team members to support clinical excellence.

Responsibilities

• Conduct comprehensive evaluations to assess residents’ physical, cognitive, and psychosocial function.
• Develop individualized treatment plans focused on improving ADLs, self-care, mobility, and safety.
• Implement OT interventions including therapeutic exercises, ADL training, adaptive equipment use, and task-specific activities.
• Monitor and document residents’ progress, adjusting plans as needed to support outcomes.
• Educate residents, families, and caregivers on plans, home exercises, and safety strategies.
• Collaborate with interdisciplinary team to ensure coordinated, resident-centered care.
• Maintain accurate documentation of evaluations, treatments, progress notes, and discharge planning.
• Supervise Certified Occupational Therapy Assistants (COTAs) per state guidelines.
