Job summary

Providence PACE in Beaverton, Oregon is seeking a dedicated Occupational Therapist to support the Tanasbourne center and Washington County. Qualifications

• Bachelor's degree in Occupational Therapy is required.
• Oregon OT license required upon hire.
• National Provider BLS - American Heart Association within 30 days of hire.
• Master's degree in Occupational Therapy preferred.
• OTD (Occupational Therapy Doctorate) preferred.

Responsibilities

• Perform OT evaluations and develop the OT plan of care per PACE policies.
• Implement and coordinate the OT plan of care with the Interdisciplinary Team.
• Communicate and coordinate care with outside providers, caregivers and family members.
• Collaborate to empower elders to live independently within the community.
