Job summary

Providence PACE OT position is a full-time role based in the Tanasbourne area, serving Washington County. The OT evaluates clients and develops and implements the OT plan of care in alignment with PACE policies.

You will be part of an Interdisciplinary Team coordinating with providers, caregivers, and family members to optimize community-based elder care. Ideal candidates hold a Bachelor’s in OT and Oregon OT licensure on hire, with BLS CPR within 30 days.

Qualifications

• Bachelor's Degree in Occupational Therapy required.
• Oregon OT License required on hire.
• BLS from American Heart Association within 30 days of hire.

Responsibilities

• Perform OT evaluations and develop/implement OT plan of care.
• Collaborate with Interdisciplinary Team and coordinate with outside providers, caregivers and families.
• Support Providence PACE operations at the Tanasbourne center and Washington County catchment.
