Job summary

Health Here in Massachusetts is seeking an Occupational Therapist to assess, plan, and deliver client-centered therapy in home settings. You will develop individualized care plans and provide restorative interventions to improve daily living skills.

You will collaborate with nurses, physicians, social workers and caregivers, ensure accurate documentation, and educate families on care plans and home recommendations.

Qualifications

• Must hold OT licensure and demonstrate clinical judgment in functional assessment.
• Experience with home-based or community therapy and environmental modification.
• Ability to deliver compassionate, dignity-centered interventions.
• Excellent communication and teamwork across an interdisciplinary team.

Responsibilities

• Assess functional abilities and needs to set therapy goals.
• Develop and implement individualized, person-centered care plans.
• Provide restorative OT to improve daily living skills.
• Recommend adaptive equipment and strategies to support independence.
• Evaluate and modify home environments to enhance safety.
• Collaborate with nurses, physicians, social workers, and caregivers.
• Document patient status, progress, and outcomes.
