Qualifications

• Licensed Occupational Therapist in Massachusetts.
• Graduation from ACOTE/AMA-AOTA-accredited OT program.
• CPR certification maintained.
• Valid driver's license and reliable transportation.
• Strong oral and written communication and interpersonal skills.
• Ability to work independently and manage time effectively.
• Experience with EMR systems and clinical documentation.

Responsibilities

• Make home visits to patients in designated territories.
• Evaluate functional status including ADL/IADL, muscle function and endurance.
• Assess home environment for hazards and equipment needs.
• Develop and revise OT care plans with the team.
• Communicate plan of care to PCP or referring physician.
• Train patients in daily activities and use of prosthetic/orthotic devices.
• Maintain clinical records and report changes to the physician/team.
