Job summary

Harbor Healthcare Of Ironton is seeking a compassionate Occupational Therapist (OT) to evaluate, plan, and implement individualized therapy for residents in a full-time capacity. You will collaborate with physicians, nursing, rehab staff, residents, and families to restore function and enhance independence.

Responsibilities include developing treatment plans, delivering evidence-based interventions, and documenting care in compliance with regulations.

Qualifications

Current OT license in Ohio.
Graduate of an accredited OT program.
Current CPR certification (or ability to obtain).

Responsibilities

Conduct comprehensive OT evaluations to identify residents' goals and needs.
Develop individualized treatment plans based on physician orders and assessments.
Implement evidence-based interventions to improve mobility, self-care, cognition, and daily living activities.
Monitor progress and modify plans as needed.
Educate residents and families on adaptive equipment, safety, and exercises.
Collaborate with the interdisciplinary team to ensure coordinated care.
Maintain compliant clinical documentation and participate in care plan meetings.
