  • Participate in care planning, discharge planning, and quality initiatives.

Position Summary

The Occupational Therapist (OT) is responsible for evaluating, planning, and implementing individualized occupational therapy services that help residents maximize independence and safely perform activities of daily living. Working collaboratively with physicians, nursing staff, rehabilitation professionals, residents, and families, the Occupational Therapist develops evidence-based treatment plans designed to restore function, promote independence, and enhance quality of life.

Key Responsibilities
  • Conduct comprehensive occupational therapy evaluations to identify residents' strengths, functional limitations, and rehabilitation goals.
  • Develop individualized treatment plans based on physician orders, clinical assessments, and resident-centered goals.
  • Implement evidence-based therapeutic interventions to improve functional mobility, self-care, cognition, upper extremity function, and activities of daily living.
  • Monitor resident progress and modify treatment plans as clinically appropriate.
  • Educate residents, families, and caregivers on adaptive equipment, home safety, therapeutic exercises, and techniques to maximize independence.
  • Collaborate with physicians, nursing staff, physical therapy, speech therapy, and the interdisciplinary care team to ensure coordinated resident care.
  • Maintain accurate, timely, and compliant clinical documentation in accordance with regulatory and facility requirements.
  • Participate in care plan meetings, discharge planning, and quality improvement initiatives that support positive resident outcomes.
  • Maintain compliance with all applicable federal, state, and professional practice standards.

Requirements

Required Qualifications
  • Current Occupational Therapist (OT) license in the State of Ohio.
  • Graduate of an accredited Occupational Therapy program.
  • Current CPR certification (or ability to obtain within required timeframe).
  • Strong knowledge of occupational therapy principles, rehabilitation techniques, and evidence-based practice.
  • Excellent communication, interpersonal, and organizational skills.
  • Ability to work collaboratively within an interdisciplinary healthcare team.
  • Commitment to providing compassionate, resident-centered care.
  • Ability to meet the physical demands of the position, including assisting residents with mobility and therapeutic activities.
Preferred Experience
  • Previous experience in skilled nursing, long-term care, inpatient rehabilitation, or a healthcare setting.
  • Experience working with geriatric and post-acute rehabilitation populations.
  • Knowledge of Medicare, skilled therapy documentation, and rehabilitation regulations.
  • Experience collaborating with interdisciplinary teams to improve resident outcomes.
  • Strong clinical reasoning, time management, and documentation skills.
  • Passion for helping residents achieve their highest level of function and independence.
