Job summary

IHA invites an Occupational Therapist to join the PACE program, serving older adults across day centers, nursing homes, and home-based settings. The role emphasizes restoring independence, safety, and quality of life through restorative, preventative, and maintenance therapies.

The OT will perform initial and ongoing assessments, develop care plans, implement interventions, and collaborate with the interdisciplinary team to meet participant-centered goals.

Qualifications

• Bachelor's degree in Occupational Therapy from an accredited program.
• Current state licensure as an Occupational Therapist.
• Minimum one year of experience working with frail or elderly populations preferred.
• Strong assessment, treatment planning, and documentation skills.
• Experience with interdisciplinary care teams and caregiver education.
• Proficiency with electronic medical records and Microsoft Office applications.

Responsibilities

• Conduct initial and ongoing assessments and develop individualized care plans.
• Implement therapeutic interventions to support independence and safety.
• Collaborate with the Interdisciplinary Team to achieve participant-centered goals.
• Supervise and evaluate COTAs and support competency assessments.
• Document participant progress in the medical record and monitor treatment effectiveness.
• Coordinate home modifications and adaptive equipment as needed.
