Elderwood Skilled Nursing & Rehabilitation is seeking a compassionate Occupational Therapist (OT) to join our geriatric rehab team. You will evaluate residents, design ADL and mobility-focused plans, and support independence with meaningful therapy in a collaborative setting.

Qualificações

• Graduation from an approved school of occupational therapy.
• Active state OT license.
• Two years’ OT experience preferred.
• Ability to communicate well verbally and in writing.
• CPR/BLS credential.
• Experience with geriatric/subacute populations.

Responsabilidades

• Provide skilled OT evaluations and treatments per physician orders.
• Develop and update individualized treatment plans and goals.
• Maintain timely clinical documentation.
• Collaborate with interdisciplinary teams regarding resident progress and discharge planning.
• Participate in resident care planning meetings.
• Support resident and family education initiatives.
• Mentor and provide clinical support as needed.
