Diamond Healthcare is seeking an Occupational Therapist to join our in-person team serving patients in clinics, home health, and inpatient settings. The role focuses on geriatrics, assessing needs and crafting effective treatment plans to restore independence and improve quality of life.

Responsibilities include evaluations, customized plans, and collaboration with care teams to ensure smooth discharge and continued home care.

Qualifications

• OT license in the state of practice.
• Bachelor’s or Master’s degree in Occupational Therapy from an accredited program.
• Strong knowledge of human physiology and anatomy relevant to OT.
• Geriatrics experience preferred.
• Familiarity with discharge planning and case management principles.
• Excellent communication with patients, families, and healthcare professionals.
• Ability to work independently and as part of a team.

Responsibilities

• Conduct comprehensive patient evaluations to determine therapy goals.
• Develop and implement individualized treatment plans across settings.
• Deliver interventions to improve daily living skills and functionality.
• Collaborate with interdisciplinary teams and plan for discharge.
• Maintain accurate documentation of progress and outcomes.
• Educate patients and families on treatment options and home programs.
• Coordinate care through case management activities.
