Job summary

BoldAge PACE, a provider delivering comprehensive in-home and center-based occupational therapy, seeks an Occupational Therapist to join its interdisciplinary team. The role emphasizes assessment, treatment planning, and collaboration with caregivers to enable seniors to live independently at home.

The OT will work across PACE settings, supervise COTAs, and contribute to care coordination, quality improvement, and staff training. A valid IL OT license and relevant experience are preferred.

Qualifications

• Advanced degree in Occupational Therapy from an accredited program.
• Current license to practice Occupational Therapy in Illinois.
• Two years of experience on an interdisciplinary team in hospital, nursing home or community setting preferred.
• Experience with frail or elderly populations preferred; training provided if not.
• Competency-based qualifications will be met before participant care begins.

Responsibilities

• Conduct initial and periodic in-person assessments of participants’ functional abilities and home environment.
• Recommend adaptive equipment, home modifications, and therapy services to promote safety.
• Develop, implement, and revise individualized treatment plans with IDT input.
• Deliver OT interventions across settings including PACE center, clinic, and participants’ homes.
• Educate participants and caregivers on equipment use and ADL strategies.
• Maintain timely documentation of assessments and progress per standards.
• Participate in daily IDT meetings and contribute to care planning.
• Supervise COTAs including review of care plans and ongoing mentorship.
• Train staff in safe OT practices and equipment use to enhance participant care.
• Support quality improvement and ongoing professional development.
