Job summary

Delhi-Rehabilitation-and-Nursing-Cente in New York is seeking a licensed Occupational Therapist to evaluate residents, establish care plans and deliver therapy per physician orders. The role requires adapting therapy as needed and documenting progress toward goals.

The OT will supervise assistants, educate residents and families, and coordinate with the interdisciplinary team while adhering to Medicare/Medicaid requirements and facility policy.

Qualifications

• Must have current NYS license as a Registered OT.
• Graduate of an accredited OT program.
• Geriatric experience preferred.
• Driver's license and reliable transportation.

Responsibilities

• Screen residents for rehabilitation therapy needs.
• Complete OT evaluations upon physician order.
• Establish OT Plan of Care for treatments per physician order.
• Provide OT plan of treatments and modalities.
• Modify treatment program level and techniques based on resident response.
• Document resident progress toward OT goals.
• Discharge residents from OT services when appropriate.
• Make recommendations for mobility, positioning, ROM and ADL tasks.
• Complete daily notes and re-certifications per policy.
• Supervise OTAs and unlicensed staff.
• Provide education to residents, staff and families.
