Job summary

Delhi-Rehabilitation-and-Nursing-Cente is seeking a licensed Occupational Therapist in New York to evaluate and treat residents in a skilled nursing setting. You will develop and adjust treatment plans, educate staff and families, and ensure compliance with Medicare and facility policies.

Ideal candidates have NYS OT licensure, a bachelor’s degree or higher, and geriatric experience preferred. A valid driver’s license and reliable transportation are required for this role.

Qualifications

• Licensed NYS Occupational Therapist in good standing.
• Bachelor’s degree or higher in OT or related field.
• Geriatric experience preferred.
• Current driver’s license and reliable transportation.

Responsibilities

• Screen residents for rehabilitation therapy needs.
• Complete OT evaluations upon physician order.
• Establish OT Plan of Care per physician order.
• Provide OT treatments and modalities.
• Modify treatment plan based on resident response.
• Document resident progress toward goals.
• Discharge residents from OT services when appropriate.
• Advise nursing on mobility, ROM and ADL tasks.
• Maintain required documentation and compliance with policies.
• Supervise OT aides and staff, provide education.
