Job summary

Orchard Rehabilitation and Nursing Center in New York is seeking a qualified Occupational Therapist to assess and treat residents. The role requires a NYS OT license, a Bachelor's degree or higher, and ability to work as part of a rehabilitation team.

You will perform evaluations, establish plans of care, supervise assistants, coordinate with nursing and therapy staff, and ensure compliance with Medicare/PDPM and facility policies.

Qualifications

• Must have current NYS OT license and degree.
• Geriatric experience preferred.
• Valid driver’s license and transportation.
• Ability to use computer applications.

Responsibilities

• Screen residents for rehabilitation therapy needs.
• Complete Occupational Therapy evaluations and establish Plan of Care.
• Provide OT treatments and modalities per physician order.
• Document resident progress toward OT goals and re-certifications.
• Supervise Certified Occupational Therapy Assistants and unlicensed staff.
• Participate in discharge planning with the interdisciplinary team.
• Comply with Medicare Part A PDPM and facility policies.
• Maintain confidentiality of resident information.
