Responsibilities

• Screen and evaluate patients with physical functioning disorders and/or wound care to develop care plans.
• Deliver rehabilitative, skilled, and medically necessary interventions per regulatory and clinical standards.
• Perform standing/walking, lifting, transferring, and repositioning as needed for patient care.
• Administer procedures in the rehab plan, including manual techniques, ambulation, therapies, modalities, and wound care.
• Provide consultation to patients, families, caregivers, and other providers regarding physical disorders.
• Generate discharge summaries complying with regulatory and clinical requirements.
