Job summary

Interlochen-Health-and-Rehabilitation-Cente in Arlington, TX, is seeking a dedicated Occupational Therapist (OT) to join our in-house rehab team. You will deliver person-centered care to improve residents’ independence, safety, and quality of life through meaningful daily activities and functional outcomes.

You will evaluate patients, develop individualized treatment plans, and collaborate with the interdisciplinary team to maximize ADLs and IADLs, while documenting thoroughly and complying with

Qualifications

• Active Texas Occupational Therapy license.
• Experience in SNF or long-term care preferred.
• Knowledge of ADLs and IADLs and related therapies.
• Ability to document thoroughly and adhere to regulations.

Responsibilities

• Consult with patients to evaluate impact on daily activities.
• Develop and implement individualized treatment plans.
• Maximize ADLs/IADLs function through therapy.
• Assess environments and recommend modifications.
• Provide training on adaptive equipment and safety.
• Collaborate with interdisciplinary team for care.
• Complete timely documentation per policies.
