Job summary

Renewal Rehab is seeking a Licensed Occupational Therapist to provide patient-centered care for seniors in skilled nursing and post-acute facilities. The onsite role involves evaluating residents, creating individualized treatment plans, and delivering evidence-based interventions to improve ADLs, mobility, cognition, and safety.

Documentation in EMR and collaboration with an interdisciplinary team are essential.

Qualifications

• Active OT license required (Illinois)
• Experience in geriatrics and skilled nursing settings preferred
• Ability to develop individualized treatment plans and set measurable goals
