Job summary

Renewal Rehab is seeking a Licensed Occupational Therapist to provide patient-centered care for seniors in skilled nursing and post-acute facilities. This onsite role involves evaluating residents, crafting individualized treatment plans, and delivering evidence-based interventions to enhance ADLs, mobility, cognition, and safety.

You will document in EMR, coordinate with an interdisciplinary team, and educate families and caregivers.

Qualifications

• Must be a licensed Occupational Therapist with OT credentials in good standing.
• Strong clinical assessment and intervention planning skills for seniors.
• Ability to collaborate with interdisciplinary care teams and families.

Responsibilities

• Evaluate residents in skilled nursing and post-acute settings and develop individualized treatment plans.
• Deliver evidence-based interventions to improve ADLs, mobility, cognition, and safety.
