Job summary

Renewal Rehab is seeking a Licensed Occupational Therapist to provide patient-centered care for seniors in skilled nursing and post-acute facilities. This onsite role involves evaluating residents, developing individualized treatment plans, and delivering evidence-based interventions to improve ADLs, mobility, cognition, and safety.

You will document in EMR, collaborate with an interdisciplinary team, and educate families and caregivers.

Qualifications

• Licensed OT with current state licensure.
• Experience in skilled nursing or post-acute settings preferred.
• Ability to evaluate residents and develop individualized treatment plans.
• Strong documentation using EMR and collaboration with care team.
• Ability to educate families and caregivers.

Responsibilities

• Evaluate residents to determine functional status and needs.
• Develop individualized treatment plans to address ADLs, mobility, cognition, and safety.
• Deliver evidence-based interventions to improve residents' functional outcomes.
• Document care in the EMR and communicate findings with the multidisciplinary team.
• Educate families and caregivers on home programs and safety considerations.
