Job summary

Renewal Rehab is seeking a Licensed Occupational Therapist to provide patient-centered care for seniors in skilled nursing and post-acute facilities. This onsite role involves evaluating residents, developing individualized treatment plans and delivering evidence-based interventions to improve ADLs, mobility, cognition, and safety.

You will document in EMR, collaborate with an interdisciplinary team, and educate families and caregivers.

Qualifications

• Active Occupational Therapist license in Illinois (or eligible).
• Experience in geriatric rehab or skilled nursing settings preferred.
• Strong assessment, treatment planning and documentation skills.

Responsibilities

• Evaluate residents and assess functional abilities.
• Develop individualized treatment plans.
• Deliver evidence-based interventions to improve ADLs, mobility, cognition and safety.
• Document in EMR and track progress over time.
