Job summary

Renewal Rehab seeks a Licensed Occupational Therapist to deliver patient-centered care for seniors in skilled nursing and post-acute facilities. In this onsite role, you will evaluate residents, develop individualized treatment plans, and provide evidence-based interventions to improve ADLs, mobility, cognition, and safety.

You will document in EMR, collaborate with an interdisciplinary team, and educate families and caregivers.

Qualifications

• Must hold active OT license in the state.
• Experience with geriatric populations preferred.
• Ability to collaborate with interdisciplinary teams.
• Strong documentation and care planning skills.
• Excellent communication with families and caregivers.

Responsibilities

• Evaluate residents and assess functional abilities.
