Job summary

Jobot seeks a Nurse Practitioner to provide primary care services to a senior population in a collaborative, patient-centered environment in Stockton, CA. This role emphasizes chronic disease management, care coordination, and helping older adults maintain health and independence.

The ideal candidate holds CA NP and RN licenses with 5+ years of nursing experience, especially in geriatrics or home health. Monday–Friday schedule, no weekends or after-hours call, and a comprehensive benefits

Qualifications

• Active California Nurse Practitioner license and RN license.
• Graduate of an accredited Nurse Practitioner program.
• Minimum of 5 years of nursing experience.
• At least 1 year of experience with elderly/frail population.
• Experience in primary care, geriatrics, managed care, skilled nursing, long-term care, or home health preferred.
• DEA registration within 6 months of hire.
• BLS certification.

Responsibilities

• Perform initial, annual, and ongoing patient assessments and develop care plans.
• Manage acute and chronic conditions for elderly patients.
• Collaborate with physicians, nurses, and the care team.
• Provide patient and family education on treatment plans and disease management.
• Coordinate referrals, transitions, and follow-up care.
• Maintain accurate clinical documentation.
