Job summary

Jobot is seeking a Nurse Practitioner in Bakersfield, CA to deliver primary care to a senior population within a collaborative, patient-centered setting. You will focus on chronic disease management, care coordination, and helping older adults maintain health and independence.

The role requires CA NP and RN licenses, 5+ years nursing experience, and experience with elderly or frail patients. Competitive benefits include healthcare, 401(k), PTO, and a Monday–Friday schedule.

Qualifications

• Active California NP license and RN license.
• Graduate of an accredited Nurse Practitioner program.
• Minimum of 5 years nursing experience.
• At least 1 year experience with elderly or frail population.
• Experience in primary care, geriatrics, managed care, skilled nursing, long-term care, or home health preferred.
• DEA registration within 6 months of hire.
• BLS certification required.

Responsibilities

• Perform initial, annual, and ongoing patient assessments and develop individualized care plans.
• Manage acute and chronic medical conditions for a frail and elderly population.
• Collaborate with physicians, nurses, specialists, and the interdisciplinary care team.
• Provide patient and family education regarding treatment plans and disease management.
• Coordinate specialty referrals, hospital transitions, and follow-up care.
• Maintain accurate and timely clinical documentation.
