Care Partners in California is seeking an in-clinic Nurse Practitioner with geriatric experience to evaluate patients, formulate treatment plans, and coordinate care across nursing homes and assisted living facilities.

This full-time role offers a Monday–Friday 8am–5pm schedule, no in-home visits, 5–8 patients per day, and a competitive salary of $150,000-$180,000 with open negotiation based on experience, plus comprehensive benefits including 100% covered medical, dental and vision, 401k match,

Qualifications

• Active Nurse Practitioner license in California.
• Geriatric care experience preferred.
• Current active DEA license.

Responsibilities

• Conduct physical exams, diagnostic tests, and treatments.
• Formulate and update treatment plans.
• Coordinate care across nursing homes and assisted living facilities.
• Educate staff, patients, and caregivers on healthcare issues.
