Job summary
Vitae Health Systems in the Midwest and East Coast is seeking a dedicated Nurse Practitioner to provide high-quality, patient-centered care to residents in skilled nursing facilities.
The NP will assess, diagnose, and manage acute and chronic conditions while collaborating with an interdisciplinary care team to improve outcomes and quality of life.
Compensation ranges from $110,000 to $125,000, with uncapped bonus opportunities and comprehensive benefits as part of a team-focused environment.
Qualifications
- Master’s degree in Nursing from an accredited program.
- Active APN license in the relevant state.
- Current DEA registration or eligibility to prescribe.
- Board certification as NP (FNP, AGNP).
- 2 years clinical experience preferred (geriatrics/long-term care).
- Strong assessment, diagnostic and care-planning skills.
- Ability to deliver value-based care.
Responsibilities
- Perform comprehensive health assessments and develop care plans.
- Order and interpret diagnostics; prescribe treatments.
- Monitor conditions to prevent complications and hospitalizations.
- Collaborate with physicians and interdisciplinary team.
- Participate in care plan meetings and quality improvement.
- Facilitate transitions of care across settings.
- Educate residents and families on diagnoses and plans.
- Maintain compliant documentation.
