Vitae Health Systems seeks a dedicated Nurse Practitioner to provide high‑quality, patient‑centered medical care to residents in skilled nursing facilities. The NP will assess, diagnose, and manage acute and chronic conditions while collaborating with an interdisciplinary care team to improve patient outcomes and quality of life.

Qualifications

• Master’s degree in Nursing from an accredited NP program.
• Active and unrestricted NP license in the respective state.
• Board certification as a Nurse Practitioner (e.g., FNP, AGNP).
• Current DEA registration and prescriptive authority (or eligibility).
• 2+ years of clinical experience, preferably in geriatrics or long-term care.
• Strong clinical assessment and treatment planning skills.
• Proficiency with electronic health records (EHR).

Responsibilities

• Perform comprehensive health assessments, diagnose and manage acute and chronic conditions, and develop individualized care plans for residents in a skilled nursing setting.
• Provide ongoing medical management including ordering and interpreting diagnostics, prescribing treatments, conducting medication reviews, and monitoring conditions.
• Deliver timely clinical interventions to respond to changes in patient status and reduce hospitalizations.
• Support value-based care initiatives by focusing on quality outcomes and data-driven decision-making.
• Collaborate with physicians and interdisciplinary team members to ensure coordinated care and participate in care plan meetings.
• Facilitate smooth transitions of care across settings and ensure continuity of treatment.
• Educate residents and families on diagnoses, treatment plans, and preventive care strategies.
• Maintain accurate, timely documentation while adhering to regulatory standards.
