Job summary

Qualifications
- Master's degree in Nursing from an accredited NP program.
- Board certification as a Nurse Practitioner.
- Active and unrestricted APN license in the respective state.
Responsibilities
- Perform comprehensive health assessments and manage acute and chronic conditions in SNF residents.
- Provide ongoing medical management including diagnostics, treatments, and medication reviews.
- Collaborate with interdisciplinary care team to improve outcomes and reduce hospitalizations.
- Support value-based care initiatives and data-driven quality improvement.
- Facilitate smooth transitions of care and ensure continuity across settings.
- Educate residents and families on diagnoses, treatment plans, and preventive care.
