Job summary

Vitae Health Systems is seeking a dedicated Nurse Practitioner to provide high-quality, patient-centered care to residents in skilled nursing facilities. You will assess, diagnose, and manage acute and chronic conditions working with an interdisciplinary team to improve outcomes and quality of life.

The role offers a competitive base salary with uncapped bonus potential and the option of a base salary or a variable compensation plan, plus comprehensive benefits and retirement support.

Qualifications

• Master’s degree in Nursing from an accredited NP program.
• Active NP license in the respective state.
• Board certification as NP (FNP/AGNP) and DEA registration.

Responsibilities

• Perform comprehensive health assessments and develop care plans for SNF residents.
• Provide medical management: diagnostics, treatments, medication reviews.
• Deliver timely interventions to improve outcomes and reduce hospitalizations.
• Support value-based care by reducing readmissions and optimizing costs.
• Collaborate with physicians and interdisciplinary team; participate in care plan meetings.
• Coordinate transitions of care across settings.
• Educate residents and families on diagnoses and prevention.
• Maintain compliant documentation and adhere to standards.
