Job summary

Vitae Health Systems is looking for a dedicated Nurse Practitioner to provide high‑quality, patient‑centered medical care in a skilled nursing facility. The NP will assess and manage both acute and chronic conditions, collaborating with an interdisciplinary team to enhance patient outcomes.

Qualifications

• Master's degree in Nursing from an accredited Nurse Practitioner program.
• Active and unrestricted Advanced Practice Nurse (APN) license in Illinois.
• Current DEA registration and prescriptive authority.
• 2 years of clinical experience preferred, ideally in geriatrics or long‑term care.

Responsibilities

• Perform comprehensive health assessments and develop care plans.
• Provide ongoing medical management and monitor conditions.
• Deliver timely clinical interventions based on patient status.
• Facilitate transitions of care and educate patients on treatment.
