Job summary

Curative in San Diego is seeking an Advanced Practice Provider (NP/PA) to join the PACE program, delivering primary and palliative care to frail seniors with extended visit times. You’ll work with an interdisciplinary team that handles logistics so you can focus on patient care.

You must hold a California NP/PA license with DEA and national board certification; experience in geriatrics or complex primary care is preferred. This role offers a stable schedule and meaningful patient relationships.

Qualifications

• Active California NP or PA license with DEA certificate and national board certification.
• Passion for geriatrics, complex primary care, or palliative medicine.

Responsibilities

• Deliver primary care, chronic disease management, and palliative care to complex, frail seniors.
• Participate in daily interdisciplinary team huddles to coordinate care.
• Provide clinical oversight across clinics, assisted living, and post-acute settings.
• Complete thorough clinical documentation using ICD-10/HCC standards.
