Job summary

TruHealth is seeking Nurse Practitioners and Physician Assistants to join a team-based, quality-of-life focused care model for geriatric residents. You will assess medical and mental health needs, develop proactive care plans, and ensure regulatory compliance across care settings.

A strong emphasis on collaboration and patient-centered results drives daily work. The role requires an APRN or PA license and 3 years’ experience in geriatric clinical nursing or rehab, plus EHR proficiency and MS

Qualifications

• APRN or PA license required.
• Degree from an accredited APRN or PA program.
• 3 years’ experience in clinical nursing or rehab in geriatric populations or Fee for Service.
• Electronic Health Records experience.
• Working knowledge of Microsoft Word/Outlook/Excel.

Responsibilities

• Assess patients’ medical and mental health needs and plan care.
• Ensure compliance with local, state, and federal agencies.
• Prescribe medications and order labs, diagnostics, and consultations.
• Monitor treatment response and modify plans as needed.
