Job summary

TruHealth is hiring Nurse Practitioners and Physician Assistants to join a mission-driven team serving seniors in long-term care. You will assess medical and mental health needs, develop proactive care plans, and ensure regulatory compliance within a fee-for-service model.

Candidates should have APRN or PA licensure, 3+ years of geriatric clinical experience, and proficiency with electronic health records and Microsoft Office tools. Equal employment opportunity employer.

Qualifications

• APRN or PA license required.
• Degree from an accredited APRN or PA program.
• 3 years’ experience in clinical nursing or rehab in geriatric populations and or Fee for Service experience.
• Electronic Health Records experience.
• Working knowledge of Microsoft applications, including Word, Outlook and Excel.

Responsibilities

• Assess patients’ medical and mental health needs and create a care plan that proactively manages their conditions and addresses barriers to care.
• Ensure compliance with local, state, and federal agencies related to clinical services you provide.
• Prescribe medications and ordering lab work, diagnostic procedures and consultations.
• Monitor patients’ compliance and response to their treatment and modify those plans as needed.
