Job summary

TruHealth is hiring Healthcare professionals to support seniors in long-term care under a Fee for Service model. NPs and PAs will work as team players to improve residents' quality of life, with a focus on proactive care planning and regulatory compliance.

The role requires APRN or PA licensure, 3 years in geriatrics or rehab, and experience with EHRs and MS Office; strong teamwork and patient-centered care are essential.

Qualifications

• APRN Advanced Practice RN or Physician Assistant license is required.
• Degree from an accredited APRN or PA program.
• 3 years' experience in clinical nursing or rehab in geriatric populations and/or Fee for Service experience.
• Electronic Health Records experience.
• Working knowledge of Microsoft applications including Word, Outlook and Excel.

Responsibilities

• Assess patients' medical and mental health needs and provide a care plan.
• Ensure compliance with local, state, and federal agencies related to clinical services.
• Prescribe medications and order lab work, diagnostic procedures and consultations.
• Monitor patients' compliance and response to treatment and modify plans as needed.
