Curana Health is seeking a compassionate Nurse Practitioner or Physician Assistant to provide high-quality primary care in skilled nursing and senior living facilities. You will deliver comprehensive patient care, manage chronic conditions, and coordinate with other healthcare providers to promote wellness and improve quality of life for seniors.

Qualifications

• Licensed Nurse Practitioner or Physician Assistant with current board certification in their field
• Master's degree in nursing or physician assistant studies
• Strong leadership, interpersonal, and problem-solving skills
• Excellent written and oral communication skills

Responsibilities

• Perform comprehensive patient assessments and develop care plans for residents.
• Conduct routine rounds to monitor residents’ conditions and adjust treatments as needed.
• Collaborate with facility staff, families, and other providers to coordinate care.
• Educate residents and families on disease prevention, wellness, and chronic care management.
• Maintain accurate EMR documentation and ensure regulatory compliance.
• Participate in quality improvement initiatives to enhance care and reduce readmissions
