Curana Health seeks a compassionate Nurse Practitioner or Physician Assistant to deliver high-quality primary care in skilled nursing facilities and senior living communities. You will focus on wellness, chronic disease management, and care coordination with other healthcare providers.

Qualifications

• Valid nurse practitioner or physician assistant license in state of operations and current board certification

Responsibilities

• Provide direct patient care, focusing on wellness, chronic disease management and coordination of care.
• Perform comprehensive assessments and develop treatment plans with ongoing care management.
• Coordinate with facility staff, families, and other providers to ensure comprehensive, coordinated care.
• Educate residents and families on disease prevention and wellness and management of chronic illnesses.
• Maintain EMR documentation and ensure regulatory compliance.
• Participate in quality improvement initiatives to enhance care delivery and reduce hospital readmissions.
