CareMed Health Solutions is seeking a Nurse Practitioner to provide medical services to residents in long-term care facilities. You will deliver primary and preventive care, manage acute changes, and coordinate with an interdisciplinary team.

Requirements include NH NP licensure, board certification, DEA license, and LTC experience; EMR proficiency is essential for continuous care planning and patient management.

Qualifications

• Current, unrestricted NP license in NH.
• Board certification in an NP specialty.
• DEA licensure in good standing.
• Clinical background in adult, family or geriatric care.
• Experience in long-term care settings.
• Ability to work with an EMR and interdisciplinary team.
• Ability to manage patients with complex chronic conditions.

Responsibilities

• Provide primary and preventive care for long-term care residents.
• Manage residents with acute changes in condition.
• Advanced care planning and chronic disease management.
• Prescribe medications, including controlled substances, per NH law.
• Order/review treatments and diagnostic tests.
• Communicate changes with nursing home staff regularly.
• Collaborate with primary care physicians as needed.
